Product: Private Medical Insurance Plan

Overview: This is a low-cost option for private medical insurance and offers benefits for inpatient treatment and outpatient treatment as an option. Benefit is paid to the policyholder on diagnosis and allows freedom to choose any hospital.

There are three levels of cover: Bronze, Silver and Gold.

The levels differ in the limit of inpatient benefit paid per years:

Treatment: The customer deals directly with the hospital for treatment, which minimises administration and keeps premiums low. Freedom provides the option of obtaining support in sourcing inpatient and outpatient from independent third parties, Preferred Health Care and European Medical Care.

Claims: Outpatient treatment costs are generally settled by reimbursement, once proof of payment for covered medical expenses has been received. Where practical, Freedom will settle the pre-authorised medical expenses either over the phone or online at the time of treatment, direct to the hospital. Once inpatient treatment is authorised, payment is made directly to the client, so they can choose to travel abroad or benefit from regional price differences for treatment as a self-pay client. Any savings are kept by the customer.

Ages at commencement: Minimum age is 18 attained. Maximum age is 65 (child-only policies are also available).

Age cover ceases for existing clients: If joined before age 65 years old, there is no upper age limit

Maximum age for a dependant child to be covered: Included children must be under age 21.

Maximum age for a full-time student to be covered: Included children in full-time education must be under age 25.

Premiums can be paid monthly: Yes. Premiums can be paid monthly by direct debit.

Minimum acceptable monthly premium: Minimum premium for a single adult, inpatient only, Bronze cover is £9.16 for an 18-year-old.

Premiums can be paid annually: Premiums can be paid annually by direct debit or cheque. Annual premiums attract a 6% discount.

Minimum acceptable annual premium: Minimum premium for a single adult, inpatient only, Bronze cover is £103.32 for an 18-year-old.

Premiums payment methods: Premiums can be collected monthly and annually by direct debit. Annual premiums can be paid by cheque.

Underwriting by moratorium clause: Moratorium underwriting - any medical condition that occurred during the five years prior to the inception of the Freedom plan will be excluded for the first two years the policy is in force.

Cover excluded by the moratorium may be reinstated later if the medical condition has not recurred for a continuous period of two years since the date it was last treated.

Compulsory excess applied: Compulsory excess is £100 applied to each claim.

Optional excess available: Premiums can be reduced by an optional excess.

Non-smoker rates available: A 6% increase in premiums if the policyholder is a smoker, or has smoked cigarettes, cigars, tobacco or pipes within the last three years.

Claims pre-authorisation service: Eligible claims are paid on diagnosis and therefore agreed before treatment.

Claims settled direct with hospital: Inpatient claims are paid to the policyholder.

In patient benefits: Full refund subject to annual cover maximum for:

Hospital accommodation, hospital nursing fees, medical expenses, ancillary charges and prescribed medicines and drugs

&149;Specialists fees

Surgeons, anaesthetists, physicians' fees (while in hospital)

&149;Diagnostic tests

MRI CT scans, tests, X-rays

&149;Cancer care

Oncology tests, drugs, consultants' fees including cover for chemotherapy and radiotherapy

Physiotherapy

&149;Childbirth

Pregnancy Complications (after 11 months)

&149;Operation fees covered

Full refund up to annual cover maximum for all operations under inpatient or additional outpatient cover

&149;Outpatient benefits

Outpatient treatment can be added to the policy at an additional cost. The annual limit of cover depends on the level or inpatient cover selected:

Freedom plan enables the customer to choose any hospital worldwide.
