Virgin Care and East Staffordshire Clinical Comissioning Group have announced a joint partnership called 'Improving Lives.'
The programme is designed to help patients with long-term conditions and help frail elderly residents have more control over their own care.
There is a tele-care service as part of the programme and remote monitoring people with conditions such as diabetes.
Service available include access to education and self-help groups as well as access seven days a week to support services.
Dr Charles Pidsley, chair of East Staffordshire CCG, said: "We are delighted to be working with Virgin Care to put the Improving Lives programme into practice.
"We are confident they will bring about the changes needed to improve care to local people, particularly those with long term health problems.
"We would like to thank all the patients and local people who have been involved in developing our vision for the Improving Lives programme which will make a real improvement to the lives of those with long-term conditions and those who are frail or elderly.
"We wish to thank all the organisations involved in the process, including the other shortlisted company, Optum, who have shown their commitment to improving healthcare."
